Indications for LTOT

  • Resting Hypoxaemia SPO2 <88%

Is a Small airway obstructive disease which is triggered by external allergens such as food (eg. eggs) noxious pungent smell and weather (cold or hot) and medications.

You must know two basic therapy

  • Reliever medication – bronchodilator which can reduce your narrowed breathing tube.

Ventolin – Usual cap of its darkish blue.

Once a day, it is a necessary medication and immediately relieves and can be used once, twice or thrice a day.

If you need this reliever medication quite often, you must see your family physician advice to check whether you are using the correct technique.

  • Preventer medication these medications can reduce anti-inflammatary effect of your airway to control your symptoms, anti-inflammation effect. It is necessary when you have frequent wheezing breathing episode.
  • Steroid inhaler.
  • Combined medication long acting bronchodilator + steroid inhaler (eg. Seravent)

Discuss with your family physician about your action plan. He’s going to instruct you the use of peak flow meter (i.e Apparatus to monitor the maximal or optimal level free from wheezing episode. During intact, it can give your doctor a guide whether you need admission or not.)

Avoidance of allergens is essential to prevent frequent attack.

If you must go to hospital at once or see your family physician once when your breathing heavily despite your usual puffer therapy.

Dangerous signs

  • When your lips are blue, sweating excessively.
  • When you are unable to talk and cold and clummy extremities call 911 or EMS service at Parami General Hospital

Hypertension and Diabetes

These two are long standing diseases and very common in our populations.

Even with the well structured care, targeted BP and Blood sugar control are achieved in a certain number of groups.

First & Foremost important step is you must change your life style.

Discuss with your doctor first if you have got chest discomfort or angina. He will direct u when and what exercise to start.

Life Style modification

Stop smoking is essential for reduction of BP as well as can prevent heart attack in both groups.

When it comes to alcohol, my advice is to stop drinking because some diabetes medication will interact w/z to make your diabetes worse or bring down your sugar dramatically (hypoglycemia)

Medical nutrition therapy is mandatory for good BP as well as glucose control. Take plenty of fruits & vegetables.

We are going to set up a structured diabeted care team in Parami General Hospital.
